INVITATION TO BID

April 20, 2015

  1. The Province of Zambales, through the Trust Fund (NPC-Masinloc CFTPP-DLF) Fund of CY - 2015, intends to apply the amount as reflected hereunder, being their Approved Budget for the Contract, to payment of the following, to wit :

Infra T04-2015 – Improvement of Taltal-Bani Provincial Road,       P 22,435,589.18

Masinloc (Construction of Soil Erosion Mitigating

Structures)

 

2.        Bids received in excess of the ABC shall be automatically rejected at the bid opening.

 

3.        Bidders should have completed, within ten (10) years from the date of submission and receipt of bids, a contract similar to the project.

 

4.        Bidding will be conducted through open competitive procedures using non-discretionary pass/fail criterion as specified in the Implementing Rules and Regulations (IRR) of Republic Act 9184, otherwise known as the “Government Procurement Reform Act”.

 

5.        Bidding is restricted to Filipino citizens/sole proprietorship, partnership, or organizations with at least seventy-five (75%) interest or outstanding capital stock belonging to citizens of the Philippines.

 

6.        Interested bidders may obtain further information from the BAC-Secretariat, PGSO, Provincial Capitol, Iba, Zambales and inspect the bidding documents at the above-specified address starting April 20, 2015 and until April 27, 2015, at 8:00 a.m to 5:00 p.m.

 

7.         A complete set of bidding documents may be purchased by interested bidders from the given address and upon payment of a non-refundable fee for the bidding documents of Twenty-Five Thousand Pesos ( P 25,000.00 ) to the Treasurer’s Cashier.

 

8.         The BAC of the Province of Zambales will hold a Pre-Bid Conference on April 27, 2015, 2:00 p.m. at the Conference Room, Office of the Provincial Engineer, Iba, Zambales, which shall be open only to all interested parties who have purchased the Bidding Documents.

 

9.         Bids must be delivered to the Bids and Awards Committee on or before 2:00 p.m, May 13, 2015 at the Conference Room, Office of the Provincial Engineer, Iba, Zambales. All bids must be accompanied by a Bid Security in any of the acceptable forms and in the amount as stated hereunder:

9.a. Cash, Cashier’s / Manager’s Check, Bank Draft /

Guarantee/Irrevocable LC                                                       -           Two Percent (2% of ABC)

9.b. Surety Bond                                                                        -           Five Percent (5% of ABC )

9.c. Bid-Securing Declaration

 

10.       The Provincial Government of Zambales, reserves the right to accept or reject any bid, to annul the bidding process, and to reject all bids at any time prior to contract award, without thereby incurring any liability to the affected bidder or bidders.

The Provincial Government of Zambales shall rescind the contract once the cumulative amount of liquidated

Invitation to Bid for the Supply/Delivery of Medical Supplies for use of Candelaria District Hospital (CDH), Candelaria, Zambales 

  1. The Provincial Government of Zambales, through the Trust Fund (TF) of CY – 2016, intends to apply the sum of Nine Hundred Ninety-Nine Thousand Nine Hundred Ninety-Nine Pesos and Three Centavos ( P 999,999.03 ), being the Approved Budget for the Contract (ABC) to payment under the contract for Supply and Delivery of Medical Supplies for use of Candelaria District Hospital (CDH), Candelaria, Zambales / Goods TF15-2016.

 

  1. Bids received in excess of the ABC shall automatically rejected and bud opening.

 

  1. The description of an eligible bidder is contained in the Bidding Documents, particularly, in Section II. Instructions to Bidders.

 

  1. Bidding will be conducted through open competitive bidding procedures using non-discretionary pass/fail criterion as specified in the Implementing Rules and Regulations (IRR) of Republic Act 9184 (RA 9184), otherwise known as the “Government Procurement Reform Act”.

 

Bidding is restricted to Filipino citizens/sole proprietorships, or organizations with at least sixty percent (60%) interest or outstanding capital stock belonging to citizens of the Philippines.

 

  1. Interested bidders may obtain further information from Bids and Awards Committee of the Province of Zambales and inspect the Bidding Documents at the address given below from 8:00 a.m. until 5:00 p.m., Monday to Friday.

 

  1. A complete set of Bidding Documents may be acquired by interested Bidders starting November 21, 2016 from the address below and upon payment of a non-refundable fee for the Bidding Documents pursuant to the latest Guidelines issued by the GPPB, in the amount of One Thousand Pesos ( P 1,000.00 ).

 

The BAC-Province of Zambales will hold a Pre-Bid Conference on November 29, 2016 at 9:00 a.m. at the BAC Office, PEO, Iba, Zambales, which shall be open to all interested parties.

 

  1. Bids must be delivered to the address below on or before 9:00 a.m., December 12, 2016 at the BAC Office, PEO, Iba, Zambales. All bids must be accompanied by a bid security in any of the acceptable forms and in the amount stated in ITB Clause 18.

 

Bids will be opened in the presence of the bidder’s representatives who choose to attend at the address below. Late bids shall not be accepted.

 

  1. The Bids and Awards Committee-Province of Zambales reserves the right to accept or reject any bid, to annul the bidding process, and to reject all bids at any time prior to contract award, without thereby incurring any liability to the affected bidder or bidders.
